Studio Policies and Procedures

LIABILITY WAIVER

Registration for classes signifies that you have agreed to the following statements:

 

I understand that physical exercise and activity can be strenuous and subject to risk of serious injury.  Any new physical exercise routine should first be approved by a qualified physician, and is done at your own risk.

 

I waive and release Monarch Dance Studio, it's Owner, Instructors, Staff and Volunteers, without limitation, from liability for any and all injuries which may occur as a result of:  (a) my participation in any activity, performance or class and (b) instruction, training, supervision or recommendations by my dance or fitness instructor.

 

I acknowledge that I have read Monarch Dance Studio's "Liability Waiver" and understand that it is a release of liability.  I expressly agree to release and discharge my Instructor(s) from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action against my Instructor(s) for personal injury or property damage.

 

To the extent that statute or case law does not prohibit release for negligence, this release is also for negligence on the part of the Instructor(s).  If any portion of this release from liability shall be deemed by a court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision(s) severed here from.  This release cannot be modified orally.

 

I also agree to allow Monarch Dance Studio to use photographic image or video of myself/my child (without identification) for advertising or promotional purposes and website/social media applications.

DRESS CODE

Out of respect for the art, the instructors and the other students, dancers will adhere strictly to the dress code.

All dancers under the age of 18 will wear the uniform specified for their age & dance style to every class. 

  • Ages 18 months - 3 yrs old (Mommy & Me):  Pink Dance Dress with white footed tights.  Pink leather ballet shoes (no satin slippers, and shoes may not have strings or ties).  Hair is to be pulled back in a ponytail with bangs secured back, short hair is to be secured back as best as possible.  Adults participating in class should wear non-restrictive clothing and socks.

  • Ages 3-1/2 yrs - 5 yrs old:  Purple Dance Dress with footed tights.  Pink leather ballet shoes (no satin slippers, and shoes may not have strings or ties), tan Velcro or elastic tap shoes (no buckles or ties).  Hair is to be pulled back in a ponytail with bangs secured back, short hair is to be secured back as best as possible.

  • Ages 6 - 8 yrs old: Powder Blue leotard and footed tights, matching powder blue pull on skirts may be worn for ballet but are not required.  Tap, jazz, and hip hop dancers may wear black dance pants or capris over their tights & leotard.  Contemporary dancers are encouraged to wear capris over their tights so their knees are covered for floor work.  Pink leather ballet shoes (no strings or ties), tan buckle or snap tap shoes (no lace-up/ribbon ties), black slip on jazz shoes (no lace-ups), black hip-hop sneakers, and contemporary dancers may be barefoot or if they prefer, wear tan half sandasoles.  Hair shall be pulled back into a ponytail with bangs secured back for all classes except ballet in which a proper ballet bun is required.  Short hair styles are to be secured back as best as possible.

  • Ages 9 - 12 yrs old: Mulberry Pink leotard and footed tights, matching mulberry pink pull on skirts may be worn for ballet but are not required.  Tap, jazz, and hip hop dancers may wear black dance pants or capris over their tights & leotard.  Contemporary dancers may wear convertable or footless tights, and are encouraged to wear capris over their tights so their knees are covered for floor work.  Pink leather ballet shoes (no strings or ties), tan slip on tap shoes (no lace-up/ribbon ties), black slip on jazz shoes (no lace-ups), black hip-hop sneakers, and contemporary dancers may be barefoot or if they prefer, wear tan half sandasoles.  Hair shall be pulled back into a ponytail with bangs secured back for all classes except ballet in which a proper ballet bun is required.  Short hair styles are to be secured back as best as possible.

  • Ages 13 - 17 yrs old: Any color studio leotard and footed tights, wrap skirts may be worn for ballet but are not required.  Tap, jazz, and hip hop dancers may wear black dance pants or capris over their tights & leotard.  Contemporary dancers may wear convertable or footless tights, and are encouraged to wear capris over their tights so their knees are covered for floor work.  Pink leather ballet shoes (no strings or ties), black slip on or lace-up tap shoes, black slip on jazz shoes (no lace-ups), black hip-hop sneakers, and contemporary dancers may be barefoot or if they prefer, wear tan half sandasoles.  Hair shall be pulled back into a ponytail with bangs secured back for all classes except ballet in which a proper ballet bun is required.  Short hair styles are to be secured back as best as possible.

  • Adults:  Adults are encouraged to wear a leotard and tights as we provide a safe and non-judgemental environment for all of our students, however if you are more comfortable in work-out clothing that is fine too.  Most of our adult students wear a leotard underneath a form fitting tank top and yoga pants.  Shoe requirements are as follows: Pink leather or canvas ballet shoes, black tap shoes, black jazz shoes, black hip-hop sneakers, and contemporary dancers may be barefoot or if they prefer, wear tan half sandasoles.  Pointe dancers will be fitted by a professional under the direction of the Instructor.  Hair shall be pulled back so as not to be distracting during class.

 

TUITION & PAYMENT

  • Tuition: Payments are due on the first class of every month.

  • Payment Options:  Tuition may be paid in cash or by check.  Credit card payments will only be accepted by automatic monthly charge (no one time credit card payments).  If you prefer to pay by credit or debit card, arrangements can be made for automatic monthly charges at any time during the year.

  • Multiple Class/Family Discounts: 

    • 1st Class:  Full price

    • 2nd Class:  10% discount

    • 3rd/more Classes:  20% discount

       Classes will be arranged in order from most to least expensive

       when calculating discounts.

  • Paid In Full Discounts:  Dancers paying in full for the year will receive 50% discount on the last month's tuition.

  • Late Fee: Tuition not received by the 10th of the month will result in a $10.00 late fee.  Any tuition not paid prior to the last day of the month will result in the interruption of class and/or recital participation until all tuition and fees are paid in full.

  • Bounced Checks: There is a $25.00 insufficient funds fee for all returned checks.  This fee, plus the original amount due must be paid in cash or with a credit card.  A second bounced check will result in the requirement of all future payments to be made in cash.

  • Refunds: Refunds are only available for pre-paid semester classes.  Full year classes that are paid monthly will not be issued refunds for missed classes or for withdrawal before the end of a month.  Refunds will not be issued after the start of the first class of each session, and all refunds will be issued less a 10% processing fee.

  • Recital Costume Fees:  The fee for Recital costumes and tights for all students is $60.00 per class.  Costume orders will be placed in December.  Costume fees must be paid before the Holiday break in order to guarantee delivery in time for pictures in the spring.

  • Cancellation:  2 week written notice must be given for cancellation of participation in classes.  Regular Tuition and automatic credit card charges will continue until written notice is provided.
